Veritas Health Screening Questionnaire

Have you taken your temperature prior to entering campus today AND was it less than 100.4 degrees? If No, we ask that you not enter the Veritas campus.

 

Have you maintained a temperature below 100.4F for the past 24 hours *without* the use of fever-reducing medication? If No, we ask that you not enter the Veritas campus.

In the past 10 days, have you experienced TWO or more of the following secondary symptoms? If Yes, we ask that you not enter the Veritas campus.

Have any immediate family or household members experienced any ONE of the previously described primary symptoms or TWO or more secondary symptoms in the last 14 days? If Yes, we ask that you not enter the Veritas campus.

 

Do you or an immediate family or household member have a pending COVID-19 laboratory test or have received a positive COVID-19 test result in the past 14 days? If Yes, we ask that you not enter the Veritas campus.

 

Have you been exposed or in close contact (e.g. living in same household, <6' physical distance for >15 minutes, exposed to respiratory droplets from coughing or sneezing, etc.) with someone who has tested positive for COVID-19 in the past 14 days? If Yes, we ask that you not enter the Veritas campus.
