Health Forms Due August 20

Prior to August 20, we ask each of our families to log in to the Commons to fill out two online forms for each child - the Student Medical Information Update Form and the Over the Counter Medication Permission Form, both available in the Forms box at the top of the page - and to look at the downloadable forms on the Health & Medical Resources board. Some of the forms must be signed by a physician, so don't wait until the last minute! Please click the link below for a list of the required forms and vaccinations.
